  • Visual pathway: retina → optic nerve → chiasm → optic tract → LGN → optic radiations → primary visual cortex (V1).
  • Optic nerve has four parts: intraocular (1 mm), intraorbital (25 mm), intracanalicular (9 mm), intracranial (16 mm).
  • Pupillary light reflex: afferent CN II → pretectal nucleus → bilateral Edinger-Westphal → CN III → ciliary ganglion → short ciliary nerves → sphincter pupillae.
  • Sympathetic pathway (3 neurons): hypothalamus → ciliospinal centre of Budge → superior cervical ganglion → carotid plexus → long ciliary nerves to dilator.
  • Cavernous sinus contents: CN III, IV, V1, V2, VI and internal carotid artery.

Adie Tonic Pupil
A dilated, slow-reacting pupil with sectoral palsy due to post-ganglionic parasympathetic denervation; classically constricts to dilute pilocarpine 0.125%.
Afferent Pupillary Defect (RAPD)
Asymmetric pupillary response on swinging-flashlight test indicating optic nerve dysfunction or extensive retinal disease.
Amaurosis Fugax
Transient monocular vision loss lasting seconds to minutes, usually embolic from carotid disease; warrants urgent vascular workup.
Anisocoria
Unequal pupil size; differentiated by greater anisocoria in light (efferent defect) vs dark (sympathetic / Horner).
Argyll Robertson Pupil
Small irregular pupils with light-near dissociation, classically associated with neurosyphilis.
Diplopia
Double vision; binocular (resolves with monocular occlusion) suggests strabismus, monocular suggests refractive or media cause.
Hemianopia
Visual field loss of half the field; bitemporal indicates chiasmal compression, homonymous indicates retrochiasmal lesion.
Hippus
Rhythmic spontaneous oscillations of pupil size unrelated to stimulation; usually a benign physiological phenomenon.
Horner Syndrome
Triad of mild ptosis, miosis, and ipsilateral anhidrosis from sympathetic chain interruption.
Leber Hereditary Optic Neuropathy
Mitochondrial DNA disease causing painless sequential bilateral subacute optic neuropathy in young males.
Marcus Gunn Pupil
Old term for relative afferent pupillary defect (RAPD).
Nystagmus
Involuntary rhythmic oscillation of the eyes; classified as jerk or pendular, congenital or acquired.
Ophthalmoplegia
Paralysis of one or more extraocular muscles producing limited motility and diplopia.
Optic Nerve Glioma
Childhood low-grade pilocytic astrocytoma of the optic nerve, often associated with neurofibromatosis type 1.

Determine which pupil is abnormal: greater anisocoria in light suggests efferent (parasympathetic) failure; greater in dark suggests sympathetic (Horner) failure. Then apply targeted pharmacological tests.
When is MRI mandatory in optic neuritis?
At first presentation to assess MS risk. Two or more brain lesions raise 15-year MS risk to 72% (ONTT data).
Difference between AION and optic neuritis?
AION: older, painless, altitudinal field defect, swollen pale disc. Optic neuritis: younger, painful eye movements, RAPD, often normal disc.
Best initial test for suspected GCA?
ESR and CRP, then start high-dose steroids immediately. Temporal artery biopsy within 2 weeks confirms.
